Provides permanent rental subsidies and housing tenancy services for Medicaid enrollees who are street or shelter homeless and are enrolled in or eligible for Health Home care coordination. Improves access to health services by addressing barriers through individualized service plan, skills development, home visits, escorts/supports, case conferencing, etc.
