Housing Assistance

Having housing is crucial to getting and staying healthy. We provide a variety of services to help people living with HIV who are homeless or unstably housed maintain safe and affordable housing and prevent eviction and utility shut off. Housing Retention Specialists assist you to remain in stable housing and live independently, thus allowing you to focus on health care and staying healthy.

New York State residents have the right to be protected from housing discrimination.

Who is eligible?

Anyone who is:

Living with HIV/AIDS, AND

Residing in Albany, Clinton, Columbia, Delaware, Essex, Franklin, Fulton, Greene, Hamilton, Montgomery, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Warren, or Washington Counties, AND

Homeless, at high risk of becoming homeless, or unstably housed.

-OR-

Anyone who:

Has active Medicaid coverage, AND

Is homeless as defined by HUD, AND

Has a least two chronic medical conditions, AND

Has five or more emergency room visits in the past year, OR 4 or more emergency room visits plus one inpatient hospital stay, or two or more inpatient hospital stays, AND

Resides in Albany, Clinton, Columbia, Delaware, Essex, Franklin, Fulton, Greene, Hamilton, Montgomery, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Warren, or Washington Counties, AND

Is able to be safely supported in a community-based setting.

Affirmative Outreach

Alliance for Positive Health adopts procedures to ensure that all persons who qualify for the assistance, regardless of their race, color, religion, sex, age, national origin, familial status, or handicap, know of the availability of the HOPWA program, including facilities and services accessible to persons with a handicap, and maintain evidence of implementation of the procedures.

How does housing assistance work?

Step 1: Apply

If you have a Care Manager, they can help you with your application. If you don't have a Care Coordinator, contact us and someone will get back to you as soon as possible.

Step 2: Verification

Once your application is received, we will need to inspect and assess your apartment or house. We will also need to gather tax information from your landlord and ask you to sign a release of information.

Step 3: Approval

You will receive a letter of approval or denial within two weeks of your application that will explain details and next steps.

Frequently asked questions about housing assistance

How do I apply?

If you have a Care Coordinator, ask them for help with the application. If you don't have a Care Coordinator, contact us and we will be in touch as soon as possible.

What information is required for the application?

Once you have submitted your application, we will need to inspect your apartment or house to ensure it meets Housing and Urban Development standards and assess the size. We will also need to obtain tax information from your landlord.

How will I know if I am approved?

You will receive a letter notifying you of approval or denial within two weeks of your application submission. Approvals will explain what you can expect and what your role will be. Denials will explain why you were not approved and what you may be able to do to qualify. It may just be a matter of providing additional information.

If you are in an emergency housing situation, we will move things through as quickly as possible to help avoid eviction and keep your utilities on.

Aside from assistance with bills, what else is involved?

We will regularly contact you to review your situation and budget, assist with planning ahead to ensure you can keep up with housing needs, get the required paperwork (things like proof of your portion of rent payment), and ensure that all your providers are on the same page to help you be as healthy as possible.

Will my information be confidential?

Maintaining client confidentiality is a core value of the Alliance for Positive Health. Safe housing and good health care is a partnership, so we will need some releases of information in order for you to be enrolled in the program.
