| Our HRSN Services connect eligible New York Medicaid members with social care services that are critical to promote their overall stability health. There are three types of services available, Housing Transportation and Nutrition.  Housing related services include household establishment, rent, utility assistance, home modifications such as ramps, air conditioners, or heaters etc. Clients are also able to access ongoing tenancy supports to help individuals maintain their housing, and live in safe, and healthy living environments. In addition, the HRSN program provides transportation services to assist clients with specific needs.  Case management and navigation to existing community resources, as well as nutrition services.  Nutrition services include but are not limited to pantry stocking, food prescriptions, home delivered meals and more. |
